We’re looking for people to take part in studies for Depression

However, please read this carefully before completing the form as it contains information about what you are consenting to by doing so and how we protect your privacy under UK Data Protection legislation.

All participants in a clinical trial need to meet certain criteria to take part (eligibility). If you are potentially suitable for one or more of our current trials based on your completed questionnaire, a member of the team will be in touch to discuss your symptoms and the trial options which may be available to you.

However, please note that eligibility criteria are laid down by the legal and scientific framework which governs the conduct of a trial (the Protocol) and may be quite restrictive, so we cannot guarantee that you will be able to be proceed on a current or future trial, even if you have already been diagnosed with a particular illness.

It is important to note that by completing this form you understand that you are not consenting to be enrolled on a trial; acceptance on a specific trial following assessment of your eligibility is governed by a separate trial informed consent process.

• This questionnaire is designed to obtain a comprehensive picture of your medical history, current medications, and ongoing commitments to assess if you might be suitable for one of our studies.
• Please note that as you complete this form, we automatically save the data on our secure internal database (Zoho), and you will be allocated a Zoho ID number.
• By filling out this form, you give GDPR consent for the data you provide to be held on the database indefinitely.
• Our database is accessible by SPCR staff only, and the data is used solely for the purpose of informing potential participants about relevant clinical studies.
• If you require transport or accommodation arrangements for a pre-agreed visit to the clinic, you give your consent for necessary data to be given to third parties for this purpose. We do not use profiling, and we do not sell or in any other way disseminate your data to third parties, other than as specified for study purposes.
• We only use third party vendors who have been assessed carefully by us as having sound data protection policies and practices.
• If you are not suitable for a current trial on review of the data you have provided, you have given consent for us to hold your personal data on our database so that we can get in touch should a trial arrive that suits your situation.
• Individuals may revise or remove their data at any time by contacting SPCR. However, this only applies to data which we hold in our own database. Once you enrol on a trial, your data relating to the trial belongs to the company which is developing the new medicine.
· Please note that, with your permission, we may use the secure DocuSign platform to obtain your signature, when requesting Medical Records from your GP.

Once you have filled in the form, we will communicate with you by e-mail, telephone (including text messages) or WhatsApp. All contact methods used are considered secure and only accessible via our VPN (Virtual Private Network) on computer or via security-protected work phones.
